When I was in Les Mis last year, we were doing a full dress runthrough with the setting up. I bent down during once scene and when I came back up again I hit my head really hard on the edge of the banister which caused my to fall back down, on my chin. When I got back up I had blood dripping down my face from the whack on the head, and a bruised chin, but I had to keep acting. :B

Another thing that happened was during our winter musical. We had the ensemble out on stage during a transition, singing as they were supposed to. Unfortunately, one of our tech members was bumped, and he dropped a platter full of glasses and plates onto the stage. There was a god awful crash, and half the ensemble turned around as half the tech crew whispered 'shit!' So, the line they had been singing was '1969', but it sounded like '19 sixty-shit" I don't think the audience noticed though. Anyway, the next scene was a restaraunt scene, so we sent the 'waitor' out with a broom and took care of it.
